I think Quintinshill deserved a spot on the Honorable Mention list. It occurred during WW I in Britain and involved a triple crash – a troop train collided with a local train and an express train crashed a few minutes later into debris from the troop/local crash on the adjacent track. Total death toll is believed to be 226, with the vast majority of those on the troop train. It was so traumatic that every surviving soldier was declared psychologically unfit for combat and sent home. Only 3 officers on the train were cleared for combat.
