Commissioned in 1983 Reactor No 4 has not been tested for low power(House Load) operation , while start testing suddenly reactor power go to zero , the famous Chief instructed operating personnel bring back to low power , the operators not knowing reactor dynamics ( Some time has to be given for reactor to regain control due to design defect , not at all included in the SOP manual) , immediately trying to bring back reactor to minimum load resulting unstable operation reactor power peak to level no one can measure huge thermal power , then exploded, .
The remaining 3 reactor were in operation till year 2000 is a miracle.
