A bridge in India fell into the River Ganges for the second time in a year while it was still under-construction.
The first collapse was caused by heavy rains and storms – it is unclear why it happened a second time.
Authorities have promised a full inquest into the accident.

Noticed that the fundation of the bridge was still in it formwork stage (no structure just staging for the formwork) they already load the top structure onto it. This is asking for trouble. Wonder what was the project team thinking. They should have complete the supporting pillars, do a load test before putting load on it.
