  1. We have more signage and safey measures in place at my work and we arent doing anything even remotely as dangerous. How do people with no common sense make it so far in life 😭

  2. Truck driver fail to identify feeding connection valve and stand by at all times by discharging and emergency shut off trailers valves unloading chemicals materials. I’m a truck driver I used to do these type of deliveries from to at chemicals plants all over Texas some of these facilities as a driver you’re not allowed to get out of your truck while loading or unloading some other facilities as a driver is your responsibility to load or unload the product at these facilities wearing PPE at all times I’m glad I didn’t have any incidents nor accidents I was very young and naive truck driver doing this type job hauling chemicals. I had no idea how dangerous is this job is hauling or handling chemicals. At 2019 I decided not to renew my hazardous material endorsement 🙏🏽

  3. I like how that guy jumped out of his truck. "Holy toledo, what's happening there? Oh crap, crap, crap. I know, I got it, I'll run around to the other side, and there I'll save the town. Wait, what the toledo is going on here? Crap, Crap, Crap. I'm getting out of here. Hero's don't die in green fog next to their trucks."

  4. Funny thing is we've all been there if we'ver worked in heavy industries. One time, while I was working on a curb and gutter crew, someone drove a concrete pin into a gas line. Gas was pouring out everywhere. We just stood there like a bunch of dummies. No one told us to leave the area, not even the supervisor. About ten minutes later, a fire truck showed up. They didn't clear the area either. Finally, 10 minutes later, a fire chief showed up with concern in his face and the words "EVACUATE!!!". Luckily the gas didn't ignite.

  5. Good video, i had a few immediate thoughts such as Reverse threading on the connection of one of those chemical load points and remote shut offs for both the truck facility operator,isolate ventilation in the control room. Known incidents do encourage better design.😊

  6. The way he jumps out at 3:05 and attempts to shut the supply off twice only to be pushed back and needing to retreat had me feeling like I was the one that fked things up!

    We were inches apart when they crossed our orders up at the truck stop. There was something reacting between us, and we mixed in a fantastic way. Call me, we'll turn this whole town upside down!

  8. But to be fair, the pipes were actually labelled maybe 1-2 foot away from the connection as seen on the animation and the photos. So the driver just needed to look if he´s actually connecting to the right pipe but instead he fully trusted the facility worker.

  9. I can just HEAR the “that idiot should have payed attention” and “in this industry you have to be 100% sure of what you are blah blah” chants from people.

    Or you could, I dunno, label, separate, use different couplings, have auto shutoffs, and other safeguards instead of shaming people for literally no reason.

  10. A moronic operation from beginning to end. Having the PPE locked up was criminal negligence, where do they keep the fire extinguishers, in a locked safe? The 2 guys involved should have been terminated. Working with some of the most dangerous and volatile chemicals is not a job for morons. It was just luck that the gas cloud was chlorine based, were it something like ammonia 100s would have died.

  11. Always follow the plumbing to the tank if you're unsure. And even worse, don't sit in your truck. That's plain laziness. Watch levels, pressure gauges etc. It could have been at least mitigated by either of those. I wouldn't get caught dead not doing either
